Job Description:
Overview Chestnut Health Systems is seeking three full-time
Community Health Specialists to assist in coordination of a
Regional Leadership Center project. This grant seeks to expand
services across the continuum, from Prevention, Intervention,
Treatment, and Recovery for Illinois residents in the assigned
service area. Note that the successful candidates will work
primarily from home or in the community. Chestnut Health Systems is
a leading provider of behavioral and primary health care services.
Over four decades, Chestnut has grown to over 700 committed,
compassionate employees providing substance use, mental health and
primary care treatment to patients in Central and Southern
Illinois. We provide fully integrated care for all of our patients
by combining behavioral health care services with our
community-based primary care health centers. Chestnut also conducts
research and training throughout the U.S.A. **Please note to be

application, and answer all screening questions. Responsibilities
Develop, implement, and evaluate programs intended to provide
supports, including those related to prevention, intervention,
treatment, and recovery. Lead community groups in capacity-building
and the development of evidence-based programs and practices.
Develop and maintain program budget. Conduct outreach services with
community stakeholders. Develop, conduct, and analyze needs
assessments and program evaluations. Develop, implement, and
evaluate media campaigns and/or other educational/informational
material based on current research and grant requirements. Identify
funding opportunities and coordinate, develop, and write grant and
contract applications. Provide program/grant contract reports in a
timely manner. Take a lead role in mentoring and training staff and
interns, routinely and consistently. Participate and take
leadership in local and state advocacy groups/collaborations.
Fulfill agency responsibilities including staff development,
monitoring research, participation in agency meetings and
committees and other responsibilities as designated by supervisor.
Promote the recovery model of treatment including trauma-informed
care, hope, respect, empowerment, health/wellness and
spirituality/connectedness in all interactions with others,
including utilizing recovery language. Maintain and protect
confidentiality and security of all organizational information
gained in the course of performing job responsibilities including,
but not limited to, financial and client information. Promote
Chestnut?s culture of customer service excellence through the
adherence to Chestnut?s behavioral standards for customer service.
Perform other duties as assigned, required, or specifically
negotiated with supervisor. Qualifications Position requires a
minimum of a High School diploma with at least five (5) years? of
related experience; or a Bachelor?s degree in education, public
health, or another behavioral health related field. Master?s degree
in a behavioral health, public health, or education related field
preferred. Must have experience working with youth, with knowledge
and understanding of adolescent development and school systems
preferred. Knowledge of community based prevention and community
organizing, prevention theory and strategies, and substance abuse
and related research preferred. Understanding of learning styles
and curriculum development helpful. Must have public speaking,
organizational and communication skills. Basic skills in Microsoft
Office (Word, Excel, Outlook) and Zoom required. Additional
knowledge of online reporting systems preferred. Position requires
ability to travel to various schools and other locations in the
site plan area and home internet connection. Must have a valid
driver?s license, private automobile insurance and be insurable.
Must be able to obtain DCFS CANTS Child Abuse Clearance.
Satisfactory completion of Chestnut?s background checking process.
