Overview
Chestnut Health Systems is seeking a full-time Community Health
Specialist to assist in coordination of the Leadership Center
project. This grant seeks to expand services across the continuum,
including Prevention, Intervention, Treatment, and Recovery for
Illinois residents in the assigned service area. Note that the
successful candidate will work primarily from home or in the
community.
Chestnut Health Systems is a leading provider of behavioral and
primary health care services. Over four decades, Chestnut has grown
to over 700 committed, compassionate employees providing substance
use, mental health and primary care treatment to patients in
Central and Southern Illinois. We provide fully integrated care for
all of our patients by combining behavioral health care services
with our community-based primary care health centers. Chestnut also
conducts research and training throughout the U.S.A.

Responsibilities
Develop, implement, and evaluate programs intended to provide
supports, including those related to prevention, intervention,
treatment, and recovery.
Plan and assist with grant and contract work plan development.
Utilize best practices in all aspects of grant work.
Assist in development of grant budget.
Conduct outreach services with community stakeholders.
Develop, conduct, and analyze needs assessments and program
evaluations.
Facilitate and assist organizational activities of any assigned
community group.
Develop, implement, and evaluate educational/informational material
based on current research and grant requirements.
Participate in local and state advocacy groups/collaborations.
Assume leadership role on projects as assigned by supervisor.
Fulfill agency responsibilities including staff development,
monitoring research, participation in agency meetings and
committees and other responsibilities as designated by
supervisor.
Promote the recovery model of treatment including trauma-informed
care, hope, respect, empowerment, health/wellness and
spirituality/connectedness in all interactions with others,
including utilizing recovery language.
Maintain and protect confidentiality and security of all
organizational information gained in the course of performing job
responsibilities including, but not limited to, financial and
client information.
Promote Chestnut's culture of customer service excellence through
the adherence to Chestnut's behavioral standards for customer
service.
Perform other duties as assigned, required, or specifically
negotiated with supervisor.
Qualifications
Position requires either a Bachelor's degree in education, public
health or a behavioral health related field with a minimum of two
(2) years' experience; or a Master's degree in a behavioral health
related field.
Requires intermediate skills in Microsoft Office (Word, Excel,
Outlook), Zoom and online reporting systems.
Ability to provide leadership, handle large projects, and
collaborate with others within the agency and community
preferred.
Must have experience in public speaking with organizational skills
and verbal and written communication skills.
Knowledge of community based prevention and community organizing,
prevention theory and strategies, and substance abuse and related
research preferred.
Understanding of learning styles and curriculum development
helpful.
Must have a valid driver's license, private automobile insurance
and be insurable.
Position requires ability to travel to schools and other locations
in the site plan area and home internet connection.
Must be able to obtain DCFS CANTS Child Abuse Clearance.
