Job Description:
Overview The Heart Failure nurse utilizes a systematic approach
to nursing by incorporating assessment, planning, implementation
and evaluation in the practice of nursing. Provides inpatient care
and education, working with an interdisciplinary team to coordinate
care for the heart failure and cardiomyopathy patient; including
individuals, families, and communities of the heart failure patient
population. Provides care coordination for up to 30 days post
discharge to assist with patient quality of life and the prevention
of readmission; within the RN scope of practice as defined by the
Illinois Board of Nursing. Qualifications Certifications: Basic
Life Support (BLS) within 30 days - American Heart Association

(AHA); Advanced Cardiac Life Support (ACLS) within 1 year -

(AHA)American Heart Association (AHA); Licensed Registered
Professional Nurse (RN) - Illinois Department of Financial and
Professional Regulation (IDFPR), Education: Bachelor's Degree:
Nursing; College Diploma: Nursing, Work Experience: Medical nursing
Responsibilities Assists patients in understanding diagnosis,
treatment options and resources available internal and external to
the organization. Serves as liaison in planning care between
patients, caregivers and multi-disciplinary health care providers.
Provide education and informational resources to patient to
optimize continuous and comprehensive care. Provides and
facilitates competent health care of patients and families in area
of practice. Ensures adherence to established standards of care and
scope of practice. Ensures documentation of care is complete and
accurate. Provides health and wellness education to individuals and
groups. Acts as preceptor and supports the development of other
staff and healthcare students. Provides leadership for initiatives
designed to improve the quality of care or clinical outcomes.
Assists Nursing Supervisor/coordinator in the development of
departmental standing orders. Provides detailed education to heart
failure inpatients at Bromenn medical Center. Is a vital part of
the inpatient care team for the heart failure patient population.
Attends daily inpatient huddles to provide input and address care
and educational needs for all heart failure and cardiomyopathy
patients. Provides follow up phone calls and care coordination,
post hospital discharge, to all heart failure/cardiomyopathy
patients to address ongoing needs and prevention of readmission.
Works closely with an interdisciplinary team to ensure complete
care of the heart failure patient as an inpatient and for 30 days

The compensation range for this position is $30.39per hour -
$52.27per hour. This represents a good faith minimum and maximum
range for the role at the time of posting by Carle Health. The
actual compensation offered a candidate will be dependent on a
variety of factors including, but not limited to, the candidate’s
experience, qualifications, location, training, licenses, shifts
