Requirements: MDor DO degree from an accredited medical school.
Completedresidency in Family or Internal Medicine accredited by the
AccreditationCouncil for Graduate Medical Education Boardcertified
or board eligible in Family or Internal Medicine Activeand
unrestricted medical license to practice in Arizona. CurrentDEA
registration for prescribing controlled substances. Priorexperience
