Job Description:

a licensed Social Worker (LSW/LCSW) to enhance our Social Services
team. This role involves providing top-notch social services aimed
at improving patient health outcomes, integrating clinical practice
with emotional support. At Carle Health, you will join a team
dedicated to innovative research and advanced medical treatment in
a supportive and progressive environment. The ideal candidate will
be proficient in managing a diverse caseload, conducting patient
assessments, and developing strategic intervention plans. Conduct
comprehensive patient assessments to determine psychosocial,
financial, and environmental needs. Develop and implement
personalized care plans in collaboration with healthcare teams.
Provide crisis intervention and support as needed. Advocate for
patients and their families to access services and resources.
Document all patient interactions and interventions accurately and
timely. Strong interpersonal and communication skills Ability to
assess and treat patients with a range of psychological issues
Knowledge of healthcare systems and community resources Crisis
intervention techniques Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Licensed Social Worker (LSW) or
Licensed Clinical Social Worker (LCSW) required Certification in
clinical practice preferred
