Position Summary:


$7,500.00 Sign on bonus 1 or more years of experience - External only
$5,000.00 Sign on bonus less than 1 year of experience - External only

Responsible for planning and administering physical therapy treatment plans designed to alleviate pain, restore function, prevent disabilities, and promote overall fitness and health of patients. Serves as a role model in delivery of professional services and as a clinical resource for staff and students.




Qualifications:


EDUCATIONAL REQUIREMENTS
Bachelor's Degree in Physical Therapy

CERTIFICATION & LICENSURE REQUIREMENTS
Physical Therapist (PT) License Illinois upon hire and Basic Life Support (BLS) within 30 days.

EXPERIENCE REQUIREMENTS
2 years recent experience in outpatient environment including orthopedics.

SKILLS AND KNOWLEDGE
Ability to listen attentively to patients and to explain home program instructions and limitations. Must demonstrate the knowledge and skill necessary to provide treatment appropriate to the age and needs of the patient. Knowledge of physical sciences, biological and behavioral sciences. Ability to work independently and as a team member, make sound judgements/decisions, effectively prioritize work assignments and organize and communicate work-related information. Flexibility when treating patients and ability to be creative when determining treatment plans and adaptive devices that best suit each patient's needs. Strong interpersonal skills, ability to earn the trust and respect of patients. Strong written and oral communication skills. Ability to recognize and respond to urgent/emergency situation. Strong computer skills, Microsoft and previous EMR experience helpful.





Essential Functions:


  • Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor, if applicable. Is aware of and demonstrates safe and appropriate body mechanics to reduce the risk of injury to self, staff and patients.
  • Demonstrates respect by treating our patients and each other with dignity and respect, being empathetic and caring, and staying fully engaged with those who need our services.
  • Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan. Adjusts the care based on patient response and available evidence.
  • Educates patients, family, caregivers, students, and other health care providers using relevant and effective teaching methods.
  • Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
  • Performs examination, selecting reliable and valid examination methods relevant to the chief complaint, results of screening, and history.
  • Screens patients using procedures to determine the effectiveness of and need fortherapy services, if appropriate.
  • Uses support personnel according to legal standards and ethical guidelines. Monitors the care delivered by support personnel and provides regular feedback.
  • Communicates with medical team.
  • Performs documentation according to department and practice standards.
  • Meets customer service and productivity standards.
  • Serves as clinical instructor when assigned by team lead/manager/supervisor.
  • Actively participates in team, program, department assignments and meetings.
  • Performs reporting and other duties as assigned.
